Mac OS X
6.1.72 (and prior)

Creating a camera, setting it to Orthographic, saving and locking it all ok. Do any kind of other work in the file and flick back to the saved camera and more often than not it defaults to a perspective version of it in the main viewer. The settings still say "orthographic" but the display is wrong. Unlocking it, toggling between Perspective then back to Orthographic refreshes the display...until the next time...and so on...

I have also noticed this, another camera bug is if you set, save and lock a camera, then do an "update geometry" and keep all the setting from the original import (keep location) the camera shifts left or right each time. :o ???
