Found this cool pic of the Joker . Added a bump and a tear. Creepy.

Nice effect!   Would be good to see the "before" image. 

Did you bring in the drawing as a texture map for a material, applied it to a plane, then apply a bump map for the texture?  Render, layer over the original in PS and remove small areas of the textured layer?

No photoshop except for sharpening and colorizing  . Just the pic on a plane. The plane is curved a little. Here's the original. I got the textures here http://www.flickr.com/photos/calebkimbrough/sets/72157623212295309/detail/
