Imported parts are in incorrect spot

I have an assembly and only needed to replace one part in it.  When I import a new one and select "from previous import" for coordinates, it still isn't in the correct spot.  Usually it will be slightly off in one direction, and sometimes the scale is way off (even though the scale typed in is exactly the same for both the original import and the new import under the Scene tab.

What is going on here?

i've noticed scale issues when replacing 1 element as well.
